Upper Airway Resistance Syndrome (UARS) is a form of sleep-disordered breathing where a narrow airway causes increased breathing effort and disrupted sleep, often without the oxygen drops typical of OSA. Common symptoms include unrefreshing sleep, brain fog, fatigue, sleepiness and insomnia. Home tests and 4% scoring often miss it. It’s best assessed by in-lab PSG with RERAs or AASM 1A hypopneas scored, and sometimes Pes (esophageal pressure) to measure breathing effort directly.
